Client is a 19-year old male with TBI, wheelchair bound, limited speech, hemiplegic Available Shifts: * Park City: Every day of the week, 12 hour shifts days and nights, 7-7 We are looking for an RN or LPN who is interested in the following:

• Medication set up and oversight

Provide thoughtful care to clients based on personalized Plan of Care * Provide in home assessments * Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed * Collaborate with client's family on plan of care, health changes, and concerns

• Weekdays include going to school with client
• Total personal cares

Joint Commission Accredited RN and LPN Requirements

• Must have at least 1 year nursing experience

Current unrestricted RN or LPN license in Utah * Certified CPR Certification * Negative TB skin test or chest x-ray * Must be able to pass a drug test and a background check * Licensed driver with valid automobile insurance * Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
